Susan Blosser APRN, FNP, ACHPN – Palliative Care

Susan received her Bachelor of Science in Nursing from Loyola University in Chicago, Illinois and her Master of Science in Nursing from Grand Canyon University in Phoenix, Arizona. She earned a post-master’s certificate as a family nurse practitioner from St. Francis University in Joliet, Illinois, and is certified as an Advanced Certified Hospice and Palliative Nurse by the Hospice and Palliative Nurses Association. Susan has over 16 years providing care in intensive care, hospice and palliative care settings.

Ania Kosztowski, MD – Rheumatology

Dr. Kosztowski earned her undergraduate degree from the University of Illinois Urbana – Champaign in Champaign, Illinois and her medical degree from the American University of the Caribbean School of Medicine in Cupecoy, Sint Maarten. She then completed her internal medicine residency at the Tulane University School of Medicine in Baton Rouge, Louisiana, and her fellowship in rheumatology at the Louisiana State University Health Sciences Center in New Orleans.

Andrea Valadez, MLS (ASCP)CM, PA-C – GI Hospitalist

Andrea received her Bachelor of Science degrees from Angelo State University and Texas Tech University Health Sciences Center in Lubbock. She then earned her Master of Health Science Degree in Clinical Laboratory Sciences from the University of Texas Rio Grande Valley and her Master of Medical Science Degree in Physician Assistant Studies from Yale School of Medicine in New Haven, Connecticut.

Andrea is certified by the National Commission on Certification of Physician Assistants. She will specialize in providing comprehensive care for patients with a variety of gastrointestinal and liver disorders within the hospital setting.

Clara Carter, APRN, FNP-BC – Urgent Care

Clara received her Bachelor of Science in Nursing from Arkansas Tech University in Russellville, Arkansas, and her Master of Science in Nursing – Family Nurse Practitioner Studies from Angelo State University in San Angelo. She is certified by the American Nurses Credentialing Center, and has over 11 years of experience providing care in acute care settings.

John Fardal, DO – Family Practice

Dr. Fardal received his undergraduate and graduate degrees from the University of Texas at Austin. He then earned his medical degree from the Texas College of Osteopathic Medicine at the University of North Texas Health Science Center in Fort Worth. Dr. Fardal completed his family medicine residency at the University of Texas at Austin Dell Medical School. He is certified by the American Board of Family Medicine.

Diego Castellon, MD – Hospitalist

Dr. Castellon received his undergraduate degree from Stony Brook University in Stony Brook, New York, and his medical degree from Medical University of the Americas, Saint Kitts and Nevis. He then completed his residency in internal medicine at Wyckoff Heights Medical Center and his fellowship in infectious disease at Maimonides Medical Center in Brooklyn, New York.

Dr. Castellon is certified in internal medicine and infectious disease by the American Board of Internal Medicine. He joins Shannon from Jamaica Hospital Medical Center in Queens, New York.
